After having been properly signed, sealed, and attested, this paper is to be forwarded to the clerk of the court out of which the naturalization paper was issued. State of Pennsylvania County of Northampton } SS: Demetrious George Gouvas, being first duly sworn, deposes and on his oath says: That I now reside at 539 Northampton; (Street) Easton; (City) Northampton (County) Pennsylvania; (State) that on the [blank] day of June (?), anno Domini 1910 the Court Clerk, Petersburg, Dinwiddie County, Virginia (Give title of court and location by county and State.) issued to me declaration of intention No. [blank]; that the said naturalization paper remained in my possession until on or about Nov. (Month) 1st, (Day) 1915 (Year) That on or about that date I moved to Easton from Raleigh, North Carolina, and in the process of moving my effects north, lost my first papers.
