Form 2225 Affidavit for Issuing New Naturalization Paper in Lieu of One Lost or Destroyed U.S. Department of Labor Naturalization Service None.- To be used only for paper issued on and after September 27, 1906. After having been properly signed, sealed, and attested, this paper is to be forwarded to the clerk of the court out of which the naturalization paper was issued.

State of New York} Southern District of N.Y.} SS: Alexandre Mirassyedyi, being first duly sworn, deposes and on his oath says: That I now reside at "[blank]" (street); "[blank]" (city); "[blank]" (county); "[blank]" (state); that on the 15 day of Nov, anno Domini, 1909 the Circuit Court of the United States So Dist of NY (give title of location by county and States) issued to me { declaration of intention} No. 28878; that the said naturalization paper remained in my possession until on or about "[blank]" (Month), "[blank]" (Day), 19"[blank]" (year)
